SNOMED CT International Interoperability and terminology Capability within agreed scope

SNOMED CT

We support SNOMED CT in the documented Care record and exchange scope, with terminology coverage and receiving requirements reviewed before your facility implements it.

Set by SNOMED International

What this is

SNOMED CT is a clinical terminology maintained by SNOMED International and released by national or regional release centres. It is licensed, versioned and used within a defined scope, so a name on a page says little until the edition, release and use are agreed.

What is built

  • Care diagnosis records can carry a SNOMED CT code alongside the diagnosis.
  • Clinical documentation is the linked terminology record surface.
  • Health data exchange is the linked exchange surface.
  • Terminology scope must match the agreed clinical and receiving context.

What a technical reviewer can inspect

Review the terminology used in representative clinical records and in the agreed exchange. Confirm the edition, release and permitted use with the implementation team.

Illustrative example: one recorded diagnosis

This example uses synthetic data.

A clinician records a synthetic diagnosis of hypertensive disorder for a synthetic patient. The record carries SNOMED CT concept 38341003 with the diagnosis. A health information manager reviews that the concept matches the clinician’s wording, and the exchange output is checked against what the receiving system expects. The clinician’s assessment stays with the clinician.

What to agree before implementation

  • The SNOMED CT edition and release the facility and receiving systems use.
  • The licensing position for the country and the facility’s use.
  • Who maintains the mapping between local terms and concepts, and who reviews it.
  • Which concepts the receiving system requires, tested with a representative record.

Boundaries

Terminology support is distinct from automatic clinical coding or validation of a clinician’s assessment. No automated concept extraction, universal mapping or terminology licensing entitlement is claimed.

What we claim, precisely

The system holds the evidence; the facility's accountable person attests. Nothing here asserts that a facility is compliant.

Common questions

  • No automatic concept extraction or coding is claimed. Review the documented terminology workflow and the specific concepts and receiving requirements relevant to your facility before agreeing the implementation scope.

  • This page does not state one. The edition, release and licensing position are agreed for your deployment during implementation, so confirm them with the team before relying on a specific concept set.

  • Your clinicians and health information team. Care holds the code with the record. It does not validate that a concept reflects the clinical assessment.
