NCC MERP International Clinical and safety methodology Index mapping unconfirmed

NCC MERP medication error severity index

We provide prescribing safety checks in Care. Mapping medication errors to the NCC MERP severity index is not claimed.

Set by National Coordinating Council for Medication Error Reporting and Prevention

What this is

The NCC MERP Index for Categorizing Medication Errors is guidance from the National Coordinating Council for Medication Error Reporting and Prevention in the United States. It classifies a medication error by outcome, from circumstances that could cause an error through to death. It is educational context here, not a function of the software.

What is built

  • Care e-Prescribing supports the described allergy checks.
  • Interaction checks support the prescribing review workflow.
  • NCC MERP outcome severity classification is not part of the available scope.

Why the index and the prescribing checks differ

Allergy and interaction checks warn a prescriber about a risk before a medicine is ordered. The index describes the outcome after an error has happened, and a clinical reviewer assigns the category after investigation. Recording an event and assigning a severity category are different tasks. The module link on this page identifies related medication safety scope, not an implemented index.

If you need severity classification

Review an actual incident with the team and ask how a category would be captured, assessed and reported, and by whom. Do not infer an index mapping from prescribing alerts. Agree the product evidence required before describing classification as available. Clinical assessment and incident investigation remain with the relevant professionals.

Boundaries

No NCC MERP classification engine, automatic category assignment or verified category mapping is claimed. Prescribing checks are related safety functionality only.

What we claim, precisely

The system holds the evidence; the facility's accountable person attests. Nothing here asserts that a facility is compliant.

Common questions

  • No. Allergy and interaction checks support prescribing review, while the index concerns medication error outcome severity. An explicit classification workflow and mapping have not been demonstrated.

  • No. Category assignment is not part of the available scope. A clinical reviewer assigns a category using the index, and recording an event is separate from classifying its severity.

  • Allergy and interaction checks during prescribing review, as described on the e-Prescribing page. They warn about risk before an error. The index describes the outcome after one.
