Incident management
NextHealth FlowFlow Incidents is sold separately
NXH Flow incident management brings reporting, investigation and corrective actions into one record. Staff record the incident and its harm level, flag sentinel events and assign investigators. Investigators record root cause analysis, and corrective and preventive actions are added, completed and approved before the incident is closed. The status history shows how the record moved to closure. For Abu Dhabi facilities, each incident goes to the DoH incident reporting gateway as it is reported, with maternal and perinatal death reports and the monthly zero report.

What your team can do
- Record the harm level on incident reports.
- Flag sentinel events for the relevant investigation and review.
- Assign investigators and record root cause analysis.
- Add, complete and approve corrective and preventive actions.
- Keep the status history through to closure.
- Send each incident to the DoH Abu Dhabi incident reporting gateway, with maternal and perinatal death reports and the monthly zero report.
- See each DoH submission as submitted or failed, and resubmit after a correction.
An incident needs a documented response beyond the report.
Keep the incident and its follow-up together
An incident record should help the team understand what happened and what needs to change. When the report, investigation and corrective actions are held separately, reviewers have to reconstruct the sequence before they can assess whether the response addresses the issue.
NXH Flow brings those elements into one incident workflow. Teams record the harm level, flag sentinel events and assign investigators. Root cause analysis and corrective and preventive actions keep the investigation and its follow-up with the record, and the status history shows how the incident moved to closure.
Permissions separate the roles. They cover adding a report, acting as an investigation member or lead, assigning investigators, adding, completing and approving corrective actions, and closing the report. The facility decides who holds each one, and a named person in the facility remains responsible for reviewing the response.
Illustrative example: from incident to approved action
This example uses synthetic data.
A nurse reports that a patient fell on a ward and records the harm level. A clinical risk lead assigns an investigator, who records the root cause analysis. A corrective action is added, completed by the ward manager and approved by a user who holds the approval permission. The report is then closed, and the record keeps the sequence.
For Abu Dhabi facilities: DoH incident reporting, built in
Flow is built to the Department of Health Abu Dhabi incident reporting requirements and connects to the DoH incident reporting gateway. Each incident is sent to DoH when it is reported, with its DoH level of harm, sentinel event flag, category and record status. Maternal deaths, reviewed under the national MPDSR programme, and perinatal and infant deaths have reports of their own. The monthly zero report confirms a month in which the facility recorded none, filed by a user or, if the facility switches it on, automatically. A complete incident saved as a draft can also be sent automatically after a waiting period the facility sets.
Every submission keeps the report sent and the response DoH returned, and shows as submitted or failed. A failed report can be corrected and resubmitted. The connection uses the facility’s own DoH gateway credentials, set in Flow; until they are, Flow runs in a demo mode and sends nothing. The facility’s accountable person still decides how each incident is classified, and any report outside this scope goes through the facility’s own authorised process.
Bring an illustrative incident to the demonstration and follow it from reporting to closure.

Common questions
-
Incident management sits within Flow Incidents, which is sold separately. Flow has no compulsory core package; choose the commercial modules your facility needs.
-
Yes. For Abu Dhabi facilities, Flow sends each incident to the DoH incident reporting gateway when it is reported, with its DoH level of harm and sentinel event flag. Maternal and perinatal death reports and the monthly zero report go through the same connection. Each submission shows as submitted or failed, and a failed one can be corrected and resubmitted.
-
Yes. The incident workflow supports root cause analysis and corrective and preventive actions, keeping the investigation and its follow-up with the incident record.
-
No. Flow sends the DoH incident reports described on this page, but the facility remains responsible for how each incident is classified, for any report outside that scope and for professional review.
