Commissions and payouts
NextHealth EngagePart of the Commissions and payouts add-on
NXH Engage commissions and payouts connects practitioner earnings with the invoice and the rules used to calculate them. Plans can combine percentage, flat and tiered rules by service, category or person. Commission accrues on the invoice amount net of VAT, while payout runs move from draft to paid through approvals and adjustments. Marking a run paid records the clinic’s own payment and does not transfer money. Practitioners receive statements and can view My earnings, giving everyone one record to review.

What your team can do
- Combine percentage, flat and tiered commission rules.
- Apply rules by service, category or individual practitioner.
- Accrue commission against invoices in the commercial workflow.
- Move payout runs from draft to paid through review.
- Use approvals and adjustments within the payout process.
- Provide practitioner statements and a My earnings view.
An earnings figure needs the calculation and review behind it.
Make commission calculations easier to review with the team
A practitioner’s earnings may depend on several rules, and the calculation needs to be understandable to both the clinic and the individual. The finance team also needs to distinguish accrued commission from a payout that has been reviewed and recorded as paid.
What paid means
Marking a payout run paid records that the clinic has paid it, with an optional payment reference, and moves its accruals to paid. It is an internal record. Engage does not move money to practitioners and does not connect to a bank or payroll provider, so the clinic pays by its own process and then records it. Engage also does not run payroll: salaries, statutory contributions and bank files sit outside this module. Commission is worked out on the invoice amount net of VAT.
Adjustments and reversals as built
- A manager can adjust one practitioner’s line, for example for a cash advance, with a reason, while the run is still open. Adjustments are not available once the run is locked.
- An adjustment cannot take a line below zero. If a practitioner’s reversals exceed their earnings for the period, the balance carries forward to a later run instead of being paid as a negative amount.
- When an invoice or line is voided or credited, its commission is reversed rather than deleted, so the trail shows both entries.
- A run moves from draft to locked, approved and paid. A manager with the permission to manage payout runs performs each step, and the approver is recorded.
Illustrative example: from invoice to a paid run
This example uses synthetic data.
Rania Qadir, a made-up practitioner, has a plan that pays 10 percent of the net invoice amount on facials. A facial invoiced at 400.00 net of VAT accrues 40.00 against the invoice. At month end a manager creates a payout run for the period, and it drafts one line for Rania. The manager adds a 10.00 deduction with the reason cash advance, locks the run and approves it. The clinic pays Rania through its own process, and the manager marks the run paid with the payment reference. Rania opens her statement from My earnings.
