Theatre and surgery

Take each surgical case from booking to recovery

NextHealth Care

Included in Care Hospitals; available as a day-surgery add-on to Care Clinics and Care Clinics Pro.

NXH Care theatre tools take a surgical case from booking to recovery on one record. Cases are booked into a theatre session, and Care refuses a clash on the theatre, the surgeon or the anaesthetist. The patient cannot go in until the pre-op checklist is complete. The WHO checklist is confirmed in order: sign in, time out and sign out. The anaesthesia record and operation note are frozen once signed, and implants are recorded with their lot and serial numbers.

What your team can do

  • Book cases into theatre sessions, with clashes on the theatre, surgeon or anaesthetist refused.
  • Hold the patient out of theatre until every pre-op checklist item is done or marked not applicable.
  • Confirm the WHO sign in, time out and sign out in order, with any correction added as an addendum.
  • Keep an anaesthesia record with technique, airway, timed entries and vital signs, frozen once signed.
  • Sign the operation note with findings, procedures and implants recorded by lot and serial number.
  • Score recovery with Aldrete observations before a day case goes home.
  • Bill procedures, implants, consumables, theatre time and anaesthesia time from the case.

One record for the case, from the list to recovery

A theatre list depends on the room, the team and the patient being ready at the same moment. In NXH Care a case is booked into a session for an operating room with its surgeon, assistant and anaesthetist, and Care refuses a booking that clashes on any of them or does not fit inside the session.

Safety checks in order

The pre-op checklist holds eleven items, each done or not applicable with a note, alongside the ASA class and the surgical consent. The patient cannot go into theatre while anything is missing. The WHO checklist follows in order, sign in, time out and sign out, each confirmed once with every item, and a correction is an addendum rather than an edit.

Records that do not change after signing

The anaesthesia record holds the technique, airway, induction, maintenance, timed entries, vital signs and complications. The operation note holds the findings, procedures, blood loss, specimens, drains, closure and plan, and cannot be signed before the WHO sign out. Both are frozen once signed. Implants are recorded by lot and serial number, and a search by either lists the cases that received them.

Recovery and the bill

Recovery is scored with Aldrete observations. A day case goes home only with a latest score of 9 or more and discharge instructions. Procedures, implants, consumables drawn from stock, theatre time and anaesthesia time are added to the patient’s bill.

What this module needs

Theatre and surgery is included in Care Hospitals and sold as a day-surgery add-on to Care Clinics and Care Clinics Pro. Anaesthetic drugs are recorded on the anaesthesia record rather than dispensed through Pharmacy.

Illustrative example: a day case, from booking to home

This example uses synthetic data.

A patient is booked for a laparoscopic hernia repair in the morning session. On the day the nurse completes the pre-op checklist, records the ASA class and links the signed consent, and the patient goes in. The team confirms the sign in and the time out, and the surgeon records the procedure and the mesh with its lot and serial number. After the sign out the anaesthetist signs the anaesthesia record and the surgeon signs the operation note. In recovery the Aldrete score reaches 10, discharge instructions are given and she goes home. Her bill shows the procedure, the mesh, the theatre time and the anaesthesia time.

Common questions

  • Theatre and surgery is included in Care Hospitals. Care Clinics and Care Clinics Pro can add it for day surgery.

  • Yes. Sign in, time out and sign out are confirmed in that order, each once and with every item. Procedures and implants can be added only after the time out, and the operation note cannot be signed before the sign out. The team remains responsible for carrying out the checks.

  • Each implant is recorded on the operation note with its lot and serial number and added to the bill. A search by lot or serial lists the cases that received it, which supports a recall.

  • No. Care refuses a case that clashes on the theatre, the surgeon or the anaesthetist, or that does not fit inside the session it is booked into.
