WHO surgical safety checklist
Care Theatre and surgery records the WHO surgical safety checklist on each case: sign in, time out and sign out, confirmed in order. Your clinical team carries out the checks.
Set by World Health Organization
What this is
The WHO Surgical Safety Checklist is guidance from the World Health Organization for operating theatre teams. It sets out checks before anaesthesia, before incision and before the patient leaves theatre.
What is built
- Care Theatre and surgery records the sign in, time out and sign out on each surgical case, in that order.
- Each phase is confirmed once, with every item, and a correction is recorded as an addendum.
- Procedures and implants are added only after the time out, and the operation note cannot be signed before the sign out.
- A pre-op checklist must be complete before the patient goes into theatre.
What the record shows, and what it does not
The record shows who confirmed each phase and when. It does not show how well the checks were carried out: the clinical team owns how the checklist is performed. Compare the items on screen with your local version of the checklist during the demonstration.
Boundaries
No WHO endorsement is claimed, and a completed checklist record is not evidence of a surgical outcome.
What we claim, precisely
The system holds the evidence; the facility's accountable person attests. Nothing here asserts that a facility is compliant.
Common questions
-
Yes. It is part of Care Theatre and surgery, which is included in Care Hospitals and available as a day-surgery add-on to the clinic plans.
-
No. Sign in, time out and sign out are confirmed in order, each once with every item. A correction is added as an addendum, so the original confirmation stays on record.
-
No. Care records the checklist’s three phases on each case, but no WHO endorsement is claimed, and your clinical team owns how the checklist is carried out.
