Tracer methodology

We support tracer preparation with audit findings, inspection evidence and documents, helping your team review the records behind a patient or operational journey.

What this is

Tracer methodology is a review method. Reviewers follow one patient, or one process, through an organisation and compare what is written with what actually happens. Accreditation surveyors use it, and facilities use it in their own internal reviews. It is led by people: interviews, observation and record review.

What is built

  • Clinical audits retain findings for supported audit types.
  • Facility inspections retain visit findings and photographs.
  • Document hub holds controlled versions and signatures.

What a reviewer can inspect

Your team can inspect records relevant to the journey it selects, comparing stated procedures with audit and inspection evidence. These modules provide source records. NextHealth does not provide a dedicated tracer builder, an automatic cross-module journey or a complete accreditor-specific tracer methodology.

Illustrative example: a record trail for one process

This example uses synthetic data and is not a customer case.

A facility chooses to trace how a ward manages oxygen cylinder storage. The reviewer opens the controlled procedure in the Document hub, then a synthetic clinical audit finding for that ward, then a synthetic facility inspection finding with a photograph of the storage room. The reviewer interviews the ward staff and observes the storage area. The records give the review a starting point and a comparison; the conclusions come from the people doing the tracer.

Boundaries

The facility defines the sampling, interview and observation approach. Software-generated records do not replace a tracer conducted by qualified reviewers. The module links are supporting evidence relationships, not proof of a dedicated tracer workflow or accreditor-approved methodology.

What we claim, precisely

The system holds the evidence; the facility's accountable person attests. Nothing here asserts that a facility is compliant.

Common questions

  • No dedicated tracer engine is confirmed. The linked modules hold documents, audit findings and inspection evidence that your team can use when preparing and conducting its own review.

  • People do. Qualified reviewers choose the journey, interview staff, observe practice and compare it with the records. NextHealth records can be source material, but the software does not conduct or score a tracer.

  • Controlled documents in the Document hub, clinical audit findings and facility inspection findings. Which of these your plan includes depends on the Flow modules you select.
